The Peninsula Stroke Association has proudly published this directory
since 2002 in order to assist stroke survivors and their families who may
be searching for resources and seeking help.

This directory is also a valuable resource for families, organizations and
others who provide care to members of our stroke community. The
information it contains is accurate as of September 2005, but we cannot
guarantee that changes will not have occurred by the time you use this
booklet. If you have comments regarding any of these resources, or note
inaccuracies, contact us at
support@psastroke.org.
Each of the listing offers a brief background and contact information that can quickly and accurately guide you to
programs and services to meet your needs. If you would like further assistance using this directory or finding additional
information, we encourage you to call the Peninsula Stroke Association at 650.565.8485 or email to
support@psastroke.org.

The print and production of the 2006/2007 edition of the Resource Booklet is made possible in part by a generous grant
from
The Cisco Systems Foundation and the support of our advertisers.  The reprint was made possible thats to teh
support of
Kaiser Permanante and Santa Clara Valley Medical Center.

While we greatly appreciate the information for listings and advertisements in the Resource Booklet, their
inclusion does not imply an endorsement of products or services. Please verify information and seek references
where appropriate.
